Advanced Surgical Care for Elbow Instability in Throwing Athletes
UCL reconstruction, commonly known as Tommy John surgery, is a procedure used to repair a torn or stretched ulnar collateral ligament in the elbow. This ligament is essential for stability during overhead throwing motions, particularly in baseball pitchers and other high-performance athletes.
At Michigan Orthopedic Center, our fellowship-trained orthopedic surgeons offer expert diagnosis and surgical treatment for UCL injuries. Whether you’re a competitive athlete or an active adult, we provide the care you need to restore function, reduce pain, and safely return to sport.
What Is the UCL, and Why Does It Tear?
The UCL is a band of tissue on the inner side of the elbow that helps stabilize the joint during high-stress motions. Over time, repeated forceful throws can stretch or tear the ligament, leading to pain, weakness, and a decline in performance.
Common causes of UCL injury include:
- Overuse from pitching or overhead throwing
- A single high-force injury during a throw
- Chronic stress in sports like baseball, javelin, or tennis
Symptoms That May Indicate a UCL Tear
- Pain on the inner side of the elbow during or after throwing
- Decreased velocity or control in pitching
- A popping sensation during a throw
- Elbow instability or looseness
- Numbness or tingling in the ring and pinky fingers

Who Needs UCL Reconstruction?
UCL reconstruction is typically recommended for:
- Athletes with a complete ligament tear
- Patients who do not improve with rest, therapy, or bracing
- Individuals whose goals include returning to high-level throwing or overhead activity
The UCL Reconstruction Procedure
UCL reconstruction involves replacing the damaged ligament with a graft, usually taken from the forearm, hamstring, or donor tissue. The new ligament is anchored to the bone using small tunnels or anchors to recreate the natural stability of the elbow.
The surgery is performed as an outpatient procedure, and most patients go home the same day.
Recovery and Return to Sport
Recovery is a gradual process and typically includes:
- Immobilization followed by a structured rehabilitation program
- Strengthening exercises to support the elbow and shoulder
- A gradual throwing progression under supervision
Most athletes return to competitive sports within 12 to 18 months. Your surgeon and therapy team will guide you through each phase of recovery.
